CHS Healthcare provides expert support, advice and direct delivery of individualised care services direct to the NHS, and on behalf of the NHS. Our Head Office is located in Sutton Coldfield. However, as we provide services to clients across the country, some travel and overnight stay may be required. About the role You will liaise with a range of health and social care organisations and individuals to develop a clear portrayal of care needs for Continuing Healthcare (CHC) consideration. This is a complex role completing and co-ordinating CHC assessments, involving the client, families and members of the multi-disciplinary team. Providing high quality support including complex information and analysis, communications and stakeholder management, you will work within a team of individuals supporting others to ensure services are delivered effectively. What sort of a person are we looking for? This role is ideal for someone with a professional health background e.g. nursing, physiotherapy or occupational therapy with experience and knowledge of completing assessments and reviews of care provision and patients' eligibility to NHS Continuing Healthcare and Funding Nursing Care. Ideally degree educated or with relevant professional qualifications you must be a registered health professional with recent experience in multi disciplinary assessment and continuing healthcare background. You should ideally have a working knowledge of the CHC National Framework, however, this role might be equally attractive to those persons with a transferrable skill set. We are also interested in hearing from individuals with a background in undertaking Personal Independence Payments assessments and working within the independent care home sector. What is essential is your ability to "run your own show", managing your own case load, working to tight and changing time scales. You must be outgoing, friendly with a desire to build supportive long term relationships with clients, colleagues and a wider professional network. You must pride yourself on your writing, presenting and selling skills as you will need to persuade others and move them into action. You also need strong IT, administration and analytical skills.
